DIAS has created two new forms to help parents and young people give their views as part of an EHC needs assessment. This is Me (for young people) and This is My Child (for parents and carers) aim to help you give the most useful and relevant information about
- what your child or young person’s needs are in school or college
- what works well and what challenges there are
- yours, or your young person’s, goals and aspirations for the future.
With useful tips and ideas, the forms help you explore the kind of things the 0-25 team want to know as part of an EHC needs assessment. This includes thinking about what your child is good at, what makes life enjoyable and what support your child needs to be able to learn.
You can download each form, save it on your computer, phone or tablet and then type directly into the text boxes. They’ll get bigger the more information you add.
Being involved in the decisions made about your child’s future is important, as is sharing your views. If you have a young person, the chances are they’ll want to be more and more involved in the decisions made about them too. Sharing your views as part of an EHC needs assessment is one of the most effective ways of making sure your child or young person get the support they need – so give it a go!
